"Modern Family" star Sarah Hyland was granted a temporary restraining order against her ex-boyfriend. In court documents, she claims that he choked her, pushed her and threatened to set her house on fire.

Hyland broke up with her boyfriend of five years Matthew Prokop last month. Though it was reported to be an "amicable" split, the court documents obtained by TMZ tell another story.

Hyland says last May she and Prokop got into a fight about her outfit that ended with him pinning her against a car, choking her and yelling "cunt, cunt, cunt" at her. The documents say, "His grip was so tight that I could not breathe or speak. I was scared and in fear for my life."

Hyland says she realized she was in an abusive relationship and looked to her TV mom Julie Bowen for help to try to "peacefully end the relationship." When he arrived home and saw Bowen there, Prokop ran into the backyard screaming. He threw a lighter at Hyland and that's when Bowen stepped in and told Hyland to get out of the house for her own safety.

After the break-up, Hyland says Prokop "relentlessly bombarded me with vile, threatening and emotionally disturbing texts and voice mails including his own suicide threats." He threatened to set her house on fire and vowed she would never see her dog again.

Prokop entered a rehab facility after the break-up and he got out this week. The documents for the restraining order say that it was the director of the facility who suggested that she get a restraining order because of his mental state and his attitude against her.

The temporary restraining order requires Prokop to stay at least 100 yards away from Sarah and her dog at all times.

According to Us Weekly, the pair met while they were auditioning for "High School Musical 3: Senior Year" in 2008. They also starred in Disney Channel's "Geek Charming" in 2011. Prokop made guest appearances on "Modern Family." It sounds like up until recently, they co-parented a dog named Barkley Bixby, a 4-year-old Maltipoo.
